We are excited that you have decided to participate on one of our adventures. Please take a moment and complete the appropriate travel forms for your journey.
Before you begin completing a Travel Adventure Application, you will need electronic copies of the following (you will be uploading these copies or photos):
- Passport (picture page)
- Other immigration documents if applicable
- Diving Certification card
- Dive Assure/DAN/etc. Accident Insurance Certificate of card (If you don’t have it yet, you may obtain it from links on the page.)
- Health Insurance card
- Documents for minors not traveling with both parents. See a sample here
